Theresa Eiden, MPT
In July 2000, Theresa Eiden and Sally O'Hare co-founded Chicago Area Rehabilitation Experts, Inc. with the philosophy that the community deserves quality physical therapy provided by licensed physical therapists.
Prior to becoming a physical therapist, Theresa earned her B.S. in Physical Education from Northern Illinois University. As a consultant for AT&T's employee wellness program (Total Life Concept 1987-1990), she performed EKG stress tests, designed fitness programs, organized and participated in running, cycling and triathlon clubs and more.

Though she enjoyed helping people achieve their wellness goals, Theresa wished to expand her knowledge and gain a greater understanding of the human body. She continued her education by earning a Masters in Physical Therapy from Northwestern University (1992). Since graduation, Theresa has continued to add to her knowledge-base and develop her manual skills using a wide variety of philosophies: myofascial release (John Barnes), craniosacral therapy (John Upledger), primitive reflex release (John Iams), mobilization with movement (Brian Mulligan), Body Talk (John Veltheim), muscle energy of the spine and TMJ (Mariano Rocobado), visceral manipulation (Jean-Paul Barral), women's health and more.
Because every person functions with their own unique movement patterns, Theresa enjoys integrating these various therapies to provide a personal and holistic treatment plan for each individual. The fun of combining these philosophies lies not only in the elimination of symptoms - which is just the tip of the iceberg - but in discovering and treating the root cause of dysfunction. She has always loved a good puzzle!
Theresa is blessed with the support of her husband and three children. In her free time, Theresa enjoys coaching basketball at her children's school, attempting Bikram yoga, volunteering at her church and experiencing different therapy techniques from a patient perspective. |
 | | Sally O'Hare, MPT
Sally is one of the founding partners of C.A.R.E. Physical Therapy. In the year 2000, she and Theresa Eiden began the practice with an emphasis on quality individual treatment that has never wavered. Sally received her Bachelor of Science degree from the University of Illinois in 1990 with a psychology major, and her Masters of Physical Therapy from the University of Iowa in 1992.
Treating the person and the function of their body as a whole is an important theme that underlies Sally's style of practice. Since she began her practice as a Physical Therapist in 1992, she has utilized and developed her manual therapy skills and knowledge in the outpatient orthopedic setting by working with individuals with a variety of neuromuscular and musculoskeletal dysfunctions and injuries. Her manual therapy skills include advanced training in myofascial release, visceral mobilization, craniosacral therapy, Primal Reflex Release Techniques, Body Work among others. She strives for ongoing professional development through continuing education coursework. � |

One area of specialization is working with runners and triathletes. Sally has had experience competing in numerous marathons and triathlons including finishing an Ironman triathlon herself. This experience and passion allows her to serve these patients particularly well. She has taught courses to other health professionals in "Injury Prevention" specifically for runners, and "Using Video Analysis to Prevent, Assess and Treat Running Injuries".
Sally truly enjoys the satisfaction that comes with helping challenging patients. She eagerly invites the opportunity to help individuals who have not had previous success with physical therapy or other forms of treatment.

Pamela Johnson, MSPT
Pamela has been practicing physical therapy for over twelve years and joined the team at C.A.R.E. Physical Therapy in May of 2009. In 1991, Pamela graduated with a B.S. in biology from Wake Forest University, and received her M.S. in Physical Therapy from Duke University in 1996. Since that time, Pamela has worked primarily with the orthopedic outpatient physical therapy population, focusing on manual therapy. She became a certified practitioner of Zero Balancing in 2006 and has given lectures and workshops on Zero Balancing techniques. In her spare time, Pam enjoys yoga and volunteers with Health Volunteers Overseas. |

Heather A. West, DPT, COMT, CSCS
After attending the University of Michigan for her undergraduate degree in Kinesiology, Heather obtained her Doctorate in Physical Therapy from A.T. Still University in Arizona. For the past few years, Heather’s primary focus has been outpatient orthopedics with an emphasis in manual therapy. She has completed extensive continuing education courses to further her manual skills and clinical reasoning, including instruction in craniosacral therapy techniques. In March 2008 she become a Certified Orthopedic Manual Therapist with Maitland, Australian Physiotherapy Seminars (MAPS). Heather is also a certified strength and conditioning specialist.
