Insurance Information

As a courtesy to our patients, a C.A.R.E. staff member will confirm your physical therapy benefits at the time of your first appointment. A claim will be submitted directly to your insurance carrier after each visit.

C.A.R.E. is contracted with the following managed health care plans:

  • Blue Cross Blue Shield PPO
  • Blue Choice HMO
  • CCN/First Health Network
  • Galaxy Health Network
  • Liberty Mutual Workers Comp Network
  • Medicare Part B

Out-of-Network discounts are available.

Self pay/cash discounts are available.

C.A.R.E. Physical Therapy accepts Visa and MasterCard.

Please be advised the State of Illinois requires a written prescription from your physician in order to receive treatment from a physical therapist.
